Positive Choices

Our mission is to help Medicare recipients who deal with chronic conditions set and reach their health goals, thereby reducing hospitalizations.   Following our approach, our Care Managers engage, educate and hold clients accountable. They meet clients in their homes to teach them about their health condition(s) and develop action plans to deal with triggers. Care Managers then connect clients to available community resources and follow up regularly with on-going support.   We want clients to be as healthy as they can be, in the comfort of their own homes, 52 weeks a year.

home52 Transportation

home52 Transportation seeks service providers to provide on-demand and advance-scheduled transportation services to riders with special needs. Additionally, for providers such as hospitals, health systems, dialysis clinics and federally qualified health centers, home52 Transportation can act as a centralized scheduling service, optimizing routes, schedules and vehicles to best meet the unique needs of their riders.
