home52 Transportation provides same-day and advanced scheduled
transportation for riders who need specialized transportation services.

home52 Transportation is a service of Council on Aging's wholly-owned subsidiary, home52. 

The home52 Transportation Difference

home52 Transportation is different than other transportation systems, including public transit and rideshare services. These traditional systems do not always meet the transportation needs of older adults and individuals with disabilities. We provide:

  • Door-through-door service 
  • Same-day ride availability
  • Specialized vehicles
  • GPS tracking improves safety
  • Communication with you and your appointment destination if we're running late

They need someone to collect them at their door, assist them safely into and out of the vehicle, and then to assist them into their appointment location. Who Can Ride With home52 Transportation

Residents in southwestern Ohio who need transportation to medical and other appointments can self-pay (pay out of pocket) to use home52 Transportation. 

In addition, home52 Transportation serves:

  • Hamilton County residents enrolled in certain Council on Aging programs (Elderly Services Program and FastTrack Home) 
  • Participants supported by various grants

Eligibility Guidelines

  • Live in southwestern Ohio
  • Adults only (no children)
  • Have a valid credit card on file with home52 Transportation's Passenger Portal (see below)
  • Have specialized need for transportation services. For example: require assistance from the home to the vehicle.
  • Call the coordination center for one-way trip costs

Services Offered

  • Pre-scheduled appointments to address medical, dental, vision or mental health needs
  • Same-day “sick” visits for medical, dental, vision or mental health needs
  • Regularly occurring appointments for treatment of a chronic condition
  • Pharmacy visits to pick up prescriptions
  • Trips for wellness activities
  • Stretcher service is not available for self-pay clients

Service Hours

Transportation services are available 6 am–6 pm, Monday – Friday. Weekend transportation for self-pay customers is not currently available.

Scheduling a Ride - Passenger Portal

Trips may be scheduled through home52 Transportation's Coordination Center by calling (855) 546-6352.

Riders also have the ability to schedule their own rides through home52 Transportation's Passenger Portal. A valid email address and credit card are required to create a Passenger Portal account.
