home52 Transportation is a transportation coordination center that schedules and coordinates transportation for eligible older adults and individuals with disabilities. home52 Transportation works with many transportation providers in our region to provide high quality and efficient transportation services.

Who can ride with home52 Transportation?

  • Clients enrolled in Hamilton County's Elderly Services Program (ESP) and FastTrack Home use home52 Transportation to get to medical and other appointments. Click here for more information about ESP.
  • Hamilton County residents age 55+ can self-pay to use home52 Transportation. See details below.

Self-Pay Specialized Transportation with home52

Older adults who need specialized, reliable transportation to pre-scheduled healthcare appointments, same day “sick” visits, pharmacy visits and even wellness activities, can pay privately to use home52 Transportation.

Transportation services are available 6am – 6pm, Monday – Friday. Weekend transportation for private pay customers is not currently available.

Eligibility guidelines 

  • Resident of Hamilton County
  • Age 55 or older
  • Have a valid credit card on file with home52 transportation
  • Has specialized needs for transportation services, for example needs assistance from the home to the vehicle.
  • Average cost for one-way trip: 
    •    Ambulatory service - $45/50
    •    Non-Ambulatory service - $60/70


Types of specialized transportation services

  • Pre-scheduled appointments to address medical, dental, vision, or mental health needs
  • Same day “sick” visits for medical, dental, vision or mental health needs
  • Regularly occurring appointments for treatment of a chronic condition
  • Pharmacy visits to pick up pre-ordered prescriptions
  • Trips for wellness activities
  • Stretcher service is not available for self-pay clients
